At the Family Action City and Hackney Social Prescribing and Outreach Service we support individual by offering an appointment with a Social Prescribing Coordinator who will conduct an in-depth needs assessment and then facilitate activate participation in local community based services, e.g. lunch clubs, gardening groups, benefits advice, exercise groups for the following groups of patients

  • Socially isolated  
  • Frequent attenders to GP/A+E  
  • Presenting with a social problem  
  • Mild-moderate mental health problems 
  • Keen to participate in non-clinical activities but not aware of what’s happening locally 
  • People with any long term condition

Our volunteers then support our service users in so many ways.  This could be in a befriending role offering emotional support or practical support to help them build their confidence and access different activities and services.

All of our volunteers receive training and regular supervision and support.  We ask for people to be able to commit to approx. 1-2 hours per week for a minimum of 6 months.  Volunteering with us is a great to build your confidence and experience whilst helping others.
