We are a local independent charity, established in 1965, who have been supporting older people in Camden for more than 50 years. Our role is to help those over 55 stay active, connected, independent and to empower them in later life.
The Dementia Wellbeing Service provides practical and emotional support to help older people live independently and safely at home for as long as possible, stay socially engaged and carry on doing what they love. The service supports Camden residents over the age of 55 in the early or moderate stages of dementia.
You will work one-to-one with people in their own home and in the community, providing invaluable practical and emotional support and engaging the person in activities and opportunities that interest them through their wellbeing plan.
- Calling clients on the waiting list, checking in on them and picking up on any issues or concerns to be passed on to the coordinators.
- Updating client records with information and notes provided by the coordinators.
- Researching and collating information for a client (s) at the request of the coordinators.
- Sending out information / letters etc to clients on behalf of the coordinators
- Distributing posters and information about the service to local centres and venues accessed by older people.
- Visiting clients to assist them to fulfil tasks identified and agreed by the coordinator. (e.g., help putting a filing system in place, accompanying client to an activity)
