This form is for clubs and societies to submit additional risk assessments and overnight stay details.
Please note, additional risk assessments can only completed and submitted by presidents or treasurers.
Please complete and submit this form for:
- Any new or one-off activity not covered by your core risk assessment.
- Any large scale activities involving members of the public.
- For any activity involving an overnight stay or substantial travel.
- If you have been requested to do so by the Student Activities team.
Each request will be reviewed by the Student Activities team, and senior management or UCL as appropriate. All activities will be approved on a case by case basis. The form must be submitted at least 10 working days prior to your activity.
If you are organising a trip or tour with overnight stays, you must complete the overnight stay and participant details sections of this form. Activity outside the UK mainland must be registered at least 8 weeks in advance.
Any accidents, incidents or near misses that occur while taking part in any activity must be reported online via UCL riskNET (log in and click on 'Report New Incident'). If you will be in a location without internet access, please make a note of the details, and then complete the form as soon as possible. Serious incidents such as those involving prolonged hospitalisation or loss of life should be dealt with by local emergency services, then reported to UCL communications centre via the emergency contact number 020 7679 3333 (this number is available 24 hours a day).
Students' Union UCL reserves the right to cancel any activity that does not follow this procedure or if we are not satisfied that appropriate risk management controls are in place.
