When do I use this form?
The Activity Registration Form is to be used when you want to organise a charity collection or a fundraising event for a specific charity.
Who do I return it to?
Please complete and submit the form online.
What else do I need to include with the form?
Please upload your letter of permission from the beneficiary organisation to the webform.
What is the deadline for this form?
Please submit your form a minimum of 10 working days before your event or collection.
What happens after I have emailed it in?
You will receive a confirmation email that will acknowledge that your form has been received. If any further details are required from you, a member of staff will get in touch.
