This record will be used to verify whether or not your EGM is quorate. If you have not met quoracy, your results will be passed to the Equity and Inclusion Officer for ratification. Once completed, you must upload it below.

Declaration

Please make sure all documents are fully and correctly completed. Once your results have been submitted, they will be reviewed by the Democracy and Representation Team and the Equity and Inclusion Unit. If your EGM is quorate, and has been held in line with Network Co-Option/EGM regulations your results will be ratified.
